How to submit FSA/HSA reimbursement claim for purchase that you already paid for with your Credit/Debit Card?
Short guidance in in layman’s terms:
To receive HSA/FSA reimbursement for our products, please follow these steps:
1. Submit a claim for reimbursement through your HSA/FSA administrator (e.g., Optum Financial, HSA Bank, HealthEquity, etc.)
2. Fill out the requested information from your HSA/FSA administrator, including (but not limited to):
a. The date the product was purchased or the service was rendered
b. The product description or service provided (see below description for verification of product eligibility):
*** “As of 1/9/2023, Electromedical Technologies’ products (Wellness Pro Plus and all product accessories) for external pain relief are eligible for HSA/FSA reimbursement as certified by SIGIS (Special InterestsGroup for IIAS Standards) under FLC 0127 as treatment for mixed/miscellaneous external pain relief.”
c. The provider (or store that the item was purchased from) (www.electromedtech.com)
d. The amount charged
e. The purchase receipt
f. If applicable, a statement indicating the expense has not been reimbursed from another source nor will reimbursement from another source be sought
3. Submit request for reimbursement
